ANNUAL CARE AGREEMENT — SERVICE TERMS
1. Services Provided
Hernando Senior Wellness will provide twice-weekly, 15-minute structured wellness visits to the Senior at their place of residence. Each visit includes companionship and social engagement, cognitive baseline observation and tracking, a home safety walkthrough (observational only), and a written visit report delivered to the Responsible Party.
2. Non-Clinical Service
Hernando Senior Wellness is a registered Homemaker and Companion Services (HCS) provider under Florida AHCA. All services are observational and non-clinical. We do not provide medical advice, diagnosis, treatment, or professional inspections of any kind. In a medical emergency, our staff will call 911 and notify the Responsible Party immediately.
3. Annual Plan Terms
This Agreement covers one (1) full year of twice-weekly wellness visits beginning on the enrollment date.
Monthly Payment Option: Payments are due on the same date each month. Services will be paused if a payment is missed until the account is brought current.
Full Year Prepay Option: Payment is collected in full at enrollment.
4. Cancellation Policy
Either party may cancel this Agreement with 30 days’ written notice to info@vatiyk.com.
Monthly plans: No further payments are due after the cancellation period ends.
Prepaid annual plans: A prorated refund will be issued for unused visits, minus a $75 administrative fee.
5. Scheduling
Visits are scheduled Sunday through Friday in coordination with the Senior’s routine. If a visit must be rescheduled, we will provide advance notice and reschedule within the same week where possible.
6. Responsible Party Obligations
The Responsible Party agrees to provide accurate information about the Senior’s health and care needs, notify us promptly of any significant changes in the Senior’s condition, and ensure a safe and accessible environment for our staff.
7. Confidentiality
All Senior and family information is kept strictly confidential and is never sold or shared with third parties, except as required by law.
8. Limitation of Liability
Hernando Senior Wellness’s liability under this Agreement shall not exceed the total amount paid for services in the preceding 30 days. We are not liable for outcomes resulting from pre-existing conditions or events outside our reasonable control.
9. Governing Law
This Agreement is governed by the laws of the State of Florida. Any disputes shall be resolved in Hernando County, Florida.
By completing enrollment, the Responsible Party confirms they have read, understood, and agreed to these terms.
